If you have a question regarding a referral for a fracture/acute injury, please read this page.

To ensure a smooth referral process and the best possible care for our shared patients, please include the following details in all referrals:

  • Patient Information: Full name, date of birth, and contact details
  • Relevant Medical History: Past medical conditions, previous surgeries, and current medication list
  • Diagnosis and/or Reason for Referral: A clear description of the condition, symptoms, and clinical findings
  • Imaging and Reports: Include provider details (location) for X-rays, CT scans, MRIs, or ultrasounds, along with the radiologist’s report. If the patient has physical copies or CDs of their imaging, please advise them to bring these to their appointment
  • Request for Patient Action: Please ask your patient to contact our clinic directly after the referral has been sent to schedule their appointment
  • Other Health Professionals Involved: Please provide details of any other relevant health professionals involved in the care of your patient so they may be included in correspondence from our consultations, such as allied health practitioners or other specialists
